New Patient Intake Process
Prior to making an appointment with the Serin Center, we require that you complete the steps outlined below. Once all of the steps have been completed, please call our main telephone number to schedule an appointment for all of our locations at (623) 824-5051.
Thank you for understanding that our experts are in high demand and we do our best to fill cancelled appointments to reduce our wait lists. By submitting your intake paperwork as soon as possible, you may have the opportunity to have an earlier appointment if someone else cancels. Intake paperwork can be scanned and emailed to email@example.com.
Cancellations or appointment changes must be made two full business days in advance of your scheduled appointment. In the event of a late cancellation or a no-show we will charge the appointment fee.
Insurance and Billing
The Serin Center experts are not directly contracted with insurance companies. We can provide monthly paid statements (“superbills”) for submittal for out-of-network reimbursement. We cannot guarantee reimbursement or advise you of what your insurance company will cover. For questions about insurance, please contact your insurance company directly.
Payment for Services
Full payment is due at time of service. We accept cash, checks, American Express, Discover, Master Card, Visa, HSA (Health Spending Account), and FSA (Flex Spending Account). Please consult with your HSA or FSA company for questions about using those funds for our services.

OUT-OF-STATE PATIENT ACCOMMODATION OPTIONS
Once you have received your confirmation from the Serin Center Help Desk, we invite you to book your accommodations with one of the local establishments we have partnered with. Note that the cost of your accommodations and travel is not included in services provided by Serin Center.

Being thorough on your intake forms and completing them before your scheduled appointment will help you maximize your time with your specialist. Help us help you by submitting forms by the deadline and bringing any prior medical records or important documents to your appointment. Plan to arrive a little early if it’s the first time coming to one of our locations and call us with at least 2 business days notice if you need to cancel or make a change to your appointment.
